Current Student Requests

“Students applying to college/post-secondary institutions will need their transcript released to the school in which they apply. We recommend that this form is completed at the beginning of the student’s application process. This form allows the school to send your transcript.”   

  1. Directions: In order to complete a “Current Student Transcript Release” download the application linked below, open in Adobe Reader, then click File > Save a Copy > and complete using Adobe Fill & Sign Tools. (Parents may complete the signature portion using an electronic signature.) Then submit the document via the “Guidance Department Request Form” posted on this page.

College Transcript Request:

“This form will provide specific instructions to request to send your HS Transcript to your college/post-secondary institution.”  

  1. Directions: Complete the request via the “Guidance Department Request Form” linked below.  Be prepared to supply the email address of your college/post-secondary institution.

Student Letter of Recommendation Request (Optional): 

“Students who have applied to a post-secondary institution(s) may be required to submit a letter of recommendation from their school counselor. This request will help your counselor develop a personalized letter. All students applying via the Common App or Coalition App will require a student letter of recommendation” 

  1. Directions: *Prior to requesting a letter of recommendation, please ensure that your secondary education institution and/or scholarship require a letter of recommendation from your school counselor. You do not need to submit this form unless you need a letter of recommendation. In order to complete a letter of recommendation request, download and complete the linked form below using Adobe and then submit the document via the  “Guidance Department Request Form” posted on this page.

Student Scholarship Transcript Request:

“Complete this form to request a copy of your transcript to be sent to you or a Scholarship entity to complete a Scholarship Application.” 

  1.
